New Student Registration

Please call to make an appointment to register your child.
We cannot register children after 3:30 PM.

                            Please bring the following information with you
                                    * Original birth certificate or affidavit
                                    * Proof of residence- utility bill, lease agreement, or title
                                    * Immunization record, including the following
                                                2 doses of measles vaccine
                                                3 doses of DPT, one after 4th birthday
                                                3 doses of oral polio, one after 4th birthday
                                                2 doses of MMR
                                                3 doses of hepatitis B
                                                1 dose varicella or document child had the disease (chickenpox)
                                    * Proof of physical examination
                                    * Student's social security card
